The same prediction keeps making the decision for you. The conversation will go badly. The mistake will cost more than you can tolerate. Rest will leave something important unattended. You organize the day around preventing an outcome that has not happened.
Cognitive behavioral therapy, or CBT, examines the relationship between thoughts, emotions and behavior. At The Therapist's Den, CBT-focused individual work is for women who want a clearer account of the pattern and practical ways to test a different response.

What CBT works with
A thought can influence what you notice, how you feel and what you do next. Behavior can then reinforce the thought. Avoiding a difficult conversation may provide immediate relief while leaving the original expectation untested.
CBT looks at those links in detail. It can include examining a prediction, considering the evidence, changing an avoidance pattern or practicing a behavior that serves the treatment goal.
The American Psychological Association's CBT overview describes work with both thinking and behavior. The clinical assessment determines which strategies fit a particular concern.
CBT with Clare Ende
Clare Ende, LMSW, integrates cognitive behavioral therapy with relational, trauma-informed work. Her experience includes inpatient psychiatric care, partial hospitalization and intensive outpatient treatment. She works with anxiety, depression, grief, overwhelm, mood concerns and the patterns that enter relationships.
CBT with Clare connects a specific problem to a workable plan. You might examine the prediction behind a conversation you keep avoiding, the withdrawal that is narrowing the week or the self-criticism that interrupts a reasonable decision. The work includes what happens after the appointment and how the plan needs to change when it meets ordinary life.

A practical example
Suppose you expect a reasonable request at work to be received as evidence that you cannot cope. You spend a day preparing, make the request so indirect that it is missed, then conclude that asking was pointless.
The therapist can help examine the prediction, the way you made the request and what information the outcome provides. A plan might involve a clearer request, an agreed way to observe the result and a review of what happened.
The task is connected to a real situation. It is also adjusted to the circumstances. If the workplace has repeatedly punished reasonable limits, that fact belongs in the assessment rather than being argued away as distorted thinking.
CBT for anxiety and avoidance
Avoidance can reduce discomfort in the moment and make the next attempt harder. Reassurance and repeated preparation can also become patterns that require increasing amounts of time.
CBT-informed treatment can examine the cycle and identify an appropriate way to practice something different. The plan should be specific to the anxiety concern and the person's clinical needs.
For OCD, a particular form of CBT called Exposure and Response Prevention may be required. General discussion of a fear is not interchangeable with that specialized treatment. Our OCD page explains the method-fit questions.
CBT for depression
Depression can change the way you interpret events and the amount of activity available in the day. Withdrawing from something meaningful may reduce effort immediately while making isolation or low mood harder to interrupt.
The treatment can work with those patterns in manageable steps. A plan needs to account for current energy, health, responsibilities and available support. It should not depend on turning a difficult week into another performance target.

Work between appointments
A therapist may suggest an observation, a written record or a behavior to practice. The next session should use that information. What happened when the prediction was tested? Was the task realistic? Did the plan address the actual obstacle?
Difficulty using an assignment can be useful clinical information. It may show that the task was too broad, the feared consequence was different or the amount of support was insufficient. The work should be reviewed and adjusted.
CBT is collaborative. You should be able to discuss the purpose of a task and how it connects to the concern you brought.
CBT and DBT
DBT draws on behavioral principles and adds a particular structure for working with acceptance, change, emotional intensity and skills. The two approaches overlap, but the names do not describe identical programs.
Our article on CBT and DBT explains the distinction. Your clinician can discuss method fit without requiring you to select a treatment from a list before an assessment.
The setting and amount of care
CBT appointments with Clare take place in person at 1080 East Trinity Lane, Suite 3, in East Nashville. Individual sessions are 50 minutes and $145. Routine individual appointments are out of network, with superbills available for a plan that provides reimbursement.
The practice also offers individual telehealth appointments for people located in Tennessee. Review each clinician's approach and appointment format when choosing care. Clare's CBT appointments are in person.
When symptoms repeatedly disrupt functioning between appointments, the licensed women-only IOP offers a route to discuss more structured care. The level of care and treatment approach are separate decisions within that assessment.
Questions about cognitive behavioral therapy
Is CBT just positive thinking
No. The work can examine whether a thought fits the evidence, but it also addresses behavior, context and practical consequences. Replacing a concern with an upbeat sentence is not a complete treatment plan.
Will every session involve a worksheet
The format depends on the work. A written exercise is useful when it helps identify or test a pattern. It should have a clinical purpose rather than becoming paperwork for its own sake.
